Chronic Care Management services are generally non-face-to-face services provided to those who have multiple (two or more) chronic conditions expected to last at least 12 months or for the life of the patient.

Examples of chronic conditions include, but are not limited to: Alzheimer's disease and related dementia; arthritis; asthma; atrial fibrillation; autism spectrum disorders; cancer; cardiovascular disease; Chronic Obstructive Pulmonary Disease (COPD); depression; diabetes; hypertension; and infectious diseases like HIV and AIDS. 

Services include:

  • Care coordination - At least 30 minutes a month of chronic care management services and coordination of care between your pharmacy, specialists, testing centers, hospitals and more.
  • Enhance opportunity for communication - Phone check-ins between visits to keep you on track and 24/7 access to health care professionals to keep them updated on changes in your health and answer your questions or concerns.
  • Updating your health record - Updated and regular communication with your doctor to inform them of resources you may need for support.
  • Comprehensive care management care plan - Personalized assistance from dedicated health care professionals who will work with you to create your care plans and expert assistance with setting and meeting your health goals.
